This notice is required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). In addition, medical records release and restriction forms, as well as our Patient Information document, are available.
These forms can be downloaded in a PDF format, which can be opened with Adobe Acrobat 7.0 and higher. If you do not already have Adobe Acrobat installed on your computer, or you have an outdated version, you can download the software for free at www.adobe.com. The direct link to download the Adobe Acrobat Reader is https://acrobat.adobe.com/us/en/acrobat/pdf-reader.html. Simply click on the "Free Download" button, and the website should automatically determine your computer's operating system and language (feel free to ignore the optional offers displayed by Adobe; you can uncheck those boxes before clicking the "Install Now" button). This will ensure that you have the most up-to-date version of Acrobat Reader available.
If you are downloading medical records release forms for your usage, please note that once the forms are filled out, they must be either mailed to the Mayfair office at 2600 N. Mayfair Road, Suite 901, Milwaukee, WI, 53226, or faxed to 414-778-3446. These forms will not be accepted via e-mail.

If you are having difficulty downloading these files, and would like technical assistance, please send an e-mail to firstname.lastname@example.org with the subject line "Support for HIPAA files." Please describe the problems you are having, and someone will respond to you within 24 hours.
There are two methods to access these files. The first one is to simply click on the link. A new browser will pop up with the desired file. You may either save that file using the "File" menu on your browser (choose "Save As...) or you may print it up right away. When you are done, you can close the window out and return to the HIPAA webpage.
The second method is to right-click on the link and choose the "Save Target As..." option on the menu that appears. Save the file where it will be easiest for you to access (usually your desktop), and print it up at a later date.
